Providing Innovative Food is Medicine Programs
Our Community Programs team works hand in hand with community partners to create Food is Medicine programs that reduce food insecurity for neighbors living with chronic conditions. Building on the learnings from our nutritious food delivery programs, we engage with our charitable food partners to deliver medically tailored groceries and fresh produce to under-resourced communities. Our innovative approach to Food is Medicine is hyper-localized and relationship-based, creating a more effective and sustainable solution for the entire community.

Active Food is Medicine Programs
In partnership with the Mayor’s Office for People with Disabilities, our nutritious food delivery program uses technology to coordinate services from 10 local community partners. Together, we provide 2 monthly in-home deliveries of nutritious food and fresh produce to 176 food-insecure individuals with disabilities living in Chicago’s Austin community. This pilot addresses hunger and promotes health and wellbeing for our most vulnerable neighbors.
In partnership with Sinai Chicago, one of Illinois’ largest SafetyNet hospitals, we are working with community health workers and 6 local partners to deploy technology that coordinates 2 monthly deliveries of medically tailored groceries and fresh produce. In September 2024, we launched our first cohort that serves 120 food insecure patients managing chronic disease on the west side of Chicago. This pilot is paving the way for better health outcomes and improved quality of life through nutritious food.
